One physician’s faxed order to discontinue hydrocodone, marketed as Anexsia, was misread by the pharmacist as an order to discontinue Arixtra, an anticoagulant. Another doctor intended to electronically order clonidine, an antihypertensive, but accidentally ordered the sedative clonazepam because both appeared as “CLON” on the computer screen.

It only takes a year to start draining empathy from future physicians, according to a study of medical students in the March issue of Academic Medicine. But empathy is a key quality medical schools should promote, experts said, because it makes for better physicians.

“We know as a medical community that really good communication skills with patients help the patients to comply with the instructions of the physician,” said lead study author Bruce W. Newton, PhD. “It puts them more at ease with the physician, a bond of trust is established, and if something unfortunately goes wrong, if you have this bond and communications skills, you are much less likely to be sued.”

The movement to align patient safety and payment seems to be picking up a full head of steam. Hospitals and payers are coalescing around the idea that no one should get paid for so-called never events — serious reportable events, such as wrong-site surgery, that kill or maim patients.

Perhaps most significantly, the BlueCross BlueShield Assn. announced in November 2007 that its plans will work toward ending payment for never events. The change will be phased in over several years as the Blues alters its coding and claims processes. A spokesman said adoption will vary among the 39 Blues plans, which insure more than 100 million people, because the change requires renegotiating contracts and securing agreements from local physicians and hospitals.

Emergency departments are finding it increasingly difficult to get specialists to provide on-call coverage, a growing problem that threatens patient access to quality and timely care at hospitals, a new study said.

The study, released in November by the Center for Studying Health System Change, said hospitals have struggled during the past decade to get specialists to be on call at their EDs. But hospital executives and others interviewed for the report said the situation is worsening.
